The second pharmacy in our building is Community Health Pharmacy - a pharmacy dedicated to medication packaging. It is classified as a long term care pharmacy.

This pharmacy provides services to patients that live in assisted living facilites, including adult family homes (AFH), community-based residential facilites (CBRF), and residential care apartment complexes (RCAC). Additional facilities that are serviced include county health departments, county health & human serivces & correctional facilities.

We also provide packaging for patients that are enrolled in the Family Care program (Inclusa/IRIS/MyChoice) through the State of Wisconsin.**

Types of packaging include bubble packs and strip packing. Packing duration is customizable to the patient - weekly/bi-weekly/28-day/monthly. 

We provide options for delivery of medications: pick-up/delivery/mailing.

Our working hours are Mon-Fri, 8:30am-6pm and Saturday, 8:30am-3pm.

We provide 24/7 phone service for our long-term care facilites/patients & 24/7 emergency delivery services, including holidays.

**Medication packaging is also available for patients who live independently and who are not enrolled in the Family Care program. This packaging would take place through our retail pharmacy, West Salem Pharmacy.
